in our area, code 3 means all units respond, even from other cities close by... Hostage situation, shots fired, officer down, etc

A burglary in progress would not be considered a code 3. Most of the officers would probably respond with lights and sirens until .5 miles away from the house, but still not considered code 3.

Well code 3 usually means officers respond with lights and sirens to the scene. Code 2 means lights, using the siren to clear an intersection. Code 1 is responding without lights or sirens. Code 4 means the situation is over. At least that's what it is around where I live.

Code 2 generally means (and by generally, I mean how it's used in Adam-12 and other police shows) you respond without lights or sirens, following all normal traffic laws. In LSPDFR, that is what it will mean. Code 3 is lights/sirens, but that doesn't mean you have to have the siren on the entire time you're responding. There's no need for a siren on a mile long stretch with no traffic, regardless of how bad the situation you're responding to is.

Generally you are right, however, the reason they use "Code 2" as explained in most cases as, LIGHTS, no SIREN is because it's an active scene (i.e; active silent alarm) and the officers responding don't want the suspect to know they are enroute.

When talking response codes: (most of US, however can vary by state and county)

Code 1 is normal patrol ("respond code 1" means still respond, but non-emergent, and follow normal traffic laws)

Code 2 is generally used for GET THERE QUICK but SILENT. (Lights NO siren)

Code 3 is, GET THERE!!!!! (LIGHTS and SIREN)

I also draw issue with (not trying to start a debate) "police shows", y'all should keep in mind, that's HOLLYWOOD!, hell, if you pay attention, 90+% of the "COPS" don't wear vests under their tops as everyday cops do.  Hollywood is not a true representation of real police, so PLEASE keep that in mind when you use POLICE SHOWS as an example unless it's COPS (formerly on FOX, now on SPIKE).

Code 1 is no emergency. No lights, no siren, you drive there like a normal person.
Code 2 is a semi life-threat. No lights, no siren, you drive there like a normal person. But like you're late for work.
Code 3 is a life threat. Lights and sirens for a rapid response, and allows ignoring most traffic laws (But not risk the life of others).
Code 4 is "all clear".

Public alpha testing has now started!

I think it would be great to see a fully operational Mounted Section with fully working horses as well as K-9 units allowing various dogs for different roles ie, german shephard's in quick response units, spaniel types doing sniff outs and poodles assisting with air support! well maybe not the last bit.

An off-road Motorcycle Wing/Unit would also be a good idea for the future, using possible 'Crosser' style bikes around rough deserty ground.The list is just endless, lets hope the countdown isn't!

Apparently I missed this before, or you edited it in after I read your post. But I just want to say that I'm guessing you've never watched Adam-12. It's a very accurate representation of police work during that time period (late 60s - early 70s).

It varies by department, let alone country. Every department has its own particulars about response rules. Also 10 codes aren't even universal.. Most overlap but a decent number mean different things to different locales.

Typically every responder uses judgment on how to respond unless specifically noted. So code 1 may very well mean full Rambo while 3 could be just lights on to clear intersections and then nada. There is no universal procedure nor is it binding. Department policy is just that, and there are over 18000 departments in the USA alone.
