Oh I have the same issues. Sometimes I can play for 1 minute, and other days 2 hours. I would suggest lowering the water and shadow quality as a first step, since you don't often notice the water (atleast I don't) and shadows can be set to medium or high. They are really resource heavy.

Okay, so none of those files are in the directory? Just uninstalling GTA IV doesn't remove scripts. Chances are that you have some left-over save files and these files can cause some problems. The save files are located in 2 different places. X:Users%username%AppDataLocalRockstar GamesGTA IVsavegames X:Users%username%DocumentsRockstar GamesGTA IVsavegames Note: The AppData-folder is usually hidden, make sure that you can see hidden files. Note 2: “X” is the drive where you have installed GTA IV, and %username% is replaced by your username Try to move/remove them and try the game again. Also make sure that you are running both executables as administrator by going to the GTA IV folder, right click on GTAIV.exe -> Properties -> Compatibility and select 'Run this program as administrator'. Do the same exact thing to the LaunchGTAIV.exe.

Very good suggestions for train simulators mate, I've used all of which you listed, and if I have to choose, Microsoft Train Simulator is still the one I enjoy the most. I'm also playing "World of Subways Vol 3, London Underground" which is a solid train simulator, not a environment simulator. The game looks good, but the stations, tunnels etc are pretty bland. Still, I enjoy it.

It may not be very obvious, no. A little tip: If you set a marker out on the map and call for backup, they (the backup) will spawn in your general vicinity and drive to the waypoint first.
