If the game is crashing randomly then the issue is not LCPDFR. It could be related to other mods you have installed, or the general instability of GTA IV. I say this because there are no errors listed in your LCPDFR log for the most recent game session.

You should complete the first mission first. LCPDFR should open the entire map for you regardless of how far in the game you are.

Unfortunately GTA IV is not very stable with mods installed. The best way to play for long periods of time is to play on a vanilla game - no car mods, no ped mods, only LCPDFR. Your best bet would be to reinstall GTA IV and then only install LCPDFR.

It is the main issue, yes, but I would suggest you to keep all of the files that LCPDFR installs (when copying over ELS, the only file you should "Copy and replace" is visualsettings.dat in the common folder - do not copy anything else if it replaces something).

Read the topic: There's a known issue where the installer doesn't replace the old AdvancedHook.dll with the new one.
