Do you have other audio mods replacing the vanilla GTA IV dispatcher? Are there any audio bugs/glitches during normal gameplay without mods, or elsewhere on your PC in general?

If you have Menyoo installed, removing it will resolve this issue. This is the Bermuda Triangle bug, it occurs when running Menyoo with RPH/LSPDFR.

How did you launch GTA V? Because you don't have a RPH log in your directory. That makes me wonder whether or not you were simply launching GTA V, instead of launching it through RPH. Have you tried to increase the "CWeaponInfoBlob" and "CWeaponComponentInf" values in WeaponLimitAdjuster.ini? If that didn't work, since it is a paid mod and we have no idea what the installation process looks like, make sure you've followed the exact instructions provided in readme.
