Try the game. And if it does work without mods, there's most likely a conflict. (Most likely your ELS-cars who are too high poly).

Sounds like your saves might be corrupted. The save files are located in 2 different places. X:Users%username%AppDataLocalRockstar GamesGTA IVsavegames X:Users%username%DocumentsRockstar GamesGTA IVsavegames Delete or move them and try the game again.

Can you post a screenshot of your GTA IV folder, please?

I think your files are corrupted, but that would make the game crash... What happens if you overwrite the file you have with the one I provided?

I think it's as simple as copying the game folder and trying to launch the game.
