Does your game work if you launch it without RPH? Also your RPH is out of date (you want version 1.84, not 1.83), although this wouldn't cause a crash before plugins are loaded.

What do you mean mission failed? You'll need to complete the GTA V starting story mission first.

Rename mods folder to _mods. This will disable your OpenIV mods. If this fixes it, your problem is located there and you'll either have to fix whatever is wrong with it or recreate the folder.

v2189 is the latest patch. Update your copy of GTA.
