I've heard a lot about how unstable and crashey it is, but I still wanted to try it because it seems more realistic than GTA 5.

If I run LCPDFR, custom cars and sirens, ELS and CryENB, how crashy is the game going to be? Without adding in things like 3rd party callouts etc.

LCPDFR itself is very stable. Adding third-party mods will make GTA IV crash.

Custom cars and sirens will degrade IV's performance and cause crashes. IV itself is not very stable, adding 3D modifications tends to make it even more so.

Should also be noted that GTA IV is 500% more stable on OS Win 10 haven't had a single texture crash since upgrading

GTA IV is an unstable game by default. LCPDFR is very stable by default. For me to make it enjoyable I must add a lot of mods. These are vehicles with ELS, new weapon models with updated sounds, uniforms, and much more. I personally got my mod packs from a community that I'm part of. They tested things thoroughly before releasing them. This means all the mods I have are stable with one another.

What makes my game unstable is callouts. I hate callouts anyway and could spend hours on traffic. My game crashing is very rare. I usually stop playing before it crashes and I guess I'm lucky. I didn't always have it this easy but the clan packs made it better. Also ENBs do not like my machine. It doesn't matter what they are.

LCPDFR is the best police game out there for what it offers. I still do not play GTA V much yet because it is just not up to par.

It's as stable as it's ever been in my opinion. Though, speaking about GTA 4 in general still not the most stable of a game, still very badly ported despite numerous patches.
