What are your PC or laptop specifications and game settings?

 

It's a Windows 7 64bit operating system

Processor: Intel ® Core ™ i3 CPU

Graphics Card: Intel ® HD Graphics

Memory: 4096 MB of RAM

 

And I run everything on the lowest settings

I don't know why I assumed you were using a laptop. xD Well since you're using a desktop, you can upgrade anything. You'll need to upgrade your GPU, CPU, and get some more RAM. Those things will cost some money if you're looking to play GTA at a stable frame rate.

Now if above doesn't work then here is a little help, I run GTA IV at 20-30 FPS with Intel HD and Intel Pentuim at 2.40GHz, 4 GB of ram with the Graphics DMVT at 512MB. I got a improvement after a bit of unnoticeable changes to the game, if you want it just note its in a test stage because it might not work on some.

Gave me about 3 extra FPS, not much but better than before! Thanks!

Turn off all unnecessary processes and services in task manager.  I kill everything not needed (print spool, diagnostics, windows search, IMF service, AVG, all printer/scanner services, disk defrag services...on and on).  I pickup at least 10-15fps easy doing that.  Also kill all un-needed processes...if you have like 20 processes running, that is too many.
