Thing is, the computer is not very powerful (atleast the card isn't). It's comparable to Intel HD 4000 intergrated cards, and those aren't that great. But the game has worked for you before, correct?

I'm going to have to say that it's the computer that causes the lag (and the checkpoint) to malfunction. Some users have reported reinstalling LCPDFR several times will fix the issue. And since you have followed my instructions (I hope) I don't believe I have anything more to add. My guess is it's because of how Rockstar made the PC port of GTA IV. Things break. My last tip is to delete the save files, it may have been a corruption in those. Save files are located here:
