Should I start a new and clean installation of GTA 5 in single-player once before installing or starting LSPDFR for the first time?

No, just make sure you have an unmodded and the newest gta version.

It turned out that I had to finish the very first GTA 5 Story Mode mission "Franklin and Lamar" first to be in "free mode" to make LSPDFR work.

Yeah, some of us forget to mention that you do need to pass the first couple or few missions of GTA Story Mode before LSPDFR will work.  Just a little tip, when you first start in the missions, your character will be Franklin, at that point Trevor or Micheal are not open yet.  If you play in story mode  long enough to open Trevor and you save the game with your last character saved being Trevor, that is what LSPDFR is going to see you as...kind of.  Trevor has a problem with the Lost MC's.  Whenever you drive by the Lost Garage in Mirror Park or through Stab City the Lost will automatically open fire on Trevor.  When you play LSPDFR it somehow needs to link your last saved character and if your last saved character is Trevor then when you are on duty and drive by the Lost Garage of Stab City the Lost will open fire on you as an LEO.  If you want that to happen then leave it.  If not then in the story mode switch to Franklin or Micheal.  I find Micheal being the safest character.  You might have some trouble in Ballas territory where sometimes they open fire on Franklin and they will do the same in LSPDFR if your saved character was Franklin.  If you want gangs opening fire on you as an LEO in LSPDFR then choose Trevor or Franklin.  If not then go with Micheal.  That is what I noticed about 3-4 years ago.  I doubt anything has changed so what I just explained might still be true.

Is this a random event? Because I didn't knowingly change characters.

I believe that's just something GTA V does. It switches up who you spawn as.

you can install a 100% savegame, then this shouldnt happen (if you have problems) https://de.gta5-mods.com/misc/100-save-game
