you have a bad data file in your save folder go to your start menu, type run, hit enter, then type %appdata% enter then go to local, rockstargames GTAIV and delete whats inside the save game folder. if you are using Xlive.dll this folder will be in documents rockstargames, save games

there is no guarantee to avoid errors unless you do not mod your gta at all

Some times that happens to me, the check points seem to be a glitch...fyi...there's a topic about why your asi loader may throw a false...its called how to trouble shoot through the diagnostic, and its pinned, and judging by emipre's quick responses...you might be getting some inaccurate info from him.
