I recently bought GTA IV (mainly because EFLC just crashed with mods), and for some reason i'm getting this issue while in Fullscreen and only full screen, with a black square in the top left of my screen.

Go to (C:)/Users/-Insert your windows user here-/Appdata/Rockstar Games/GTA IV/ and delete the folder called "Settings"/

If you have backups, try installing your backups.

 

Considering that i have just installed the game today, I have yet to see the point of "Backups"

Make a new .txt file in your ROOT GTA:IV folder named    "commandline.txt"

Inside it, type the line:-

-windowed

This will make GTA:IV run in windowed mode, see if that solves this problem.

 

 

Also, you might want to try Alt+Tabbing out of your game (fullscreen) AFTER you hear the initial intro music (after which you'll be on the start screen), & then Alt+Tabbing back into the game.

As i've stated its only in fullscreen mode, and i quote "i'm getting this issue while in Fullscreen and only full screen", and i Do not wish to play in Windowed mode, I've already done it and that worked.

Oh right, sorry. Should've figured you tried that already.

 

Did the Alt+Tabbing work?

Also, check to see if IV is set to your monitor's native resolution (use windowed mode to verify). (IV should've done it automatically, but maybe it goofed & is now causing this problem)
