I know that this is very stupid question.

So have i installed it wrong, because when i enter GTA IV it dosent start the mod. So how can i play the mod? Alt+P just blurs the screen.

Chances are you did not copy/overwrite some of the necessary files, or perhaps you don't have a good version of the game installed (GTA IV you should have patched to at least 1.0.6.0 or 1.0.7.0, ELFC needs to be at least 1.1.2.0)

The diagnostic tool listed in that topic will help us diagnose the issue, please run it and paste the output here.

heres an idea....are you running the game as ADMIN? if not....run launchgtaiv.exe as admin
