Put all files in the "Vehicle Files" folder into your vehicles.img.

I found vehicles.img but it is a disc file? How do I put the skins inside of it?

You need to use a program like SparkIV to open it. All the importing options are available via this program.

You can't add files to the vehicle.img manually, you can only edit it using a program such as OpenIV or SparkIV.

As stated before, if you are replacing or adding vehicles, you will need a GTA4 archive manager (I personally recommend OpenIV for vehicle modifications)

However, if you are only replacing files, there exists several vehicle mod installers. I've used this one before I knew how to use OpenIV to replace files, and it's pretty decent.
