The latest updated version of RAGE Plugin Hook, required to play LSPDFR, can be found inside the LSPDFR download. It is not currently available on the RAGE Plugin Hook website.

Set LSPDFR to not load on startup, launch the game and load it manually via the F4 console (LoadPlugin LSPDFirstResponse.dll).

If that doesn't work, delete patterns.dat and stats.dat from GTAV/lspdfr and retry.
If that still doesn't work, delete suspects.ls from the same folder and retry.
