If you use paint.net or photoshop then you can change all that yourself

It goes where everything else would your downloads unless you have it set to somewhere else

Hey gianttankblast! You can simply open the PDF form once you download it, and you can input text for any of the text boxes.

Well this is indeed a good way to keep track of those naughty NPCs..lol think ill give it a try!
