To add on to what Dimos posted, do that. It looks like you have a really outdated RPH. Also make sure when you install you do: LSPDFR and then RPH.

Have you tried going into the RPH settings and setting the game to start in forced windowed mode? Then try launching RPH again.

Otherwise it would be in the list of callouts that loaded in the log. The confusion comes from the fact that the thing that processes callouts for LSPDFR is also called CalloutManager.
@JamesSte Please remove Fire Callouts and try again.

Additionally, please provide the RPH Crash Log. Its a text file in the Main Directory called either RagePluginHook or RagePluginHook.log (if you have file extensions enabled).
