Looks to me like you just need to install EUP Serve and Rescue. You may only have the EUP Law Enforcement pack installed. 
 
 
 
 
The FiveM pack is for server owners to install and set up on the server, and only adds the vehicles and some maps, no callouts. The single player mod, including the callouts themselves, are for single player only and have nothing to do with FiveM. It sounds like you'll need to find a FiveM server to join that has the pack installed. I don't really play FiveM so I have no particular recommendations.
 
 
Looks like it is crashing when requesting an ambulance from the LSPDFR backup system. If you go to one of those locations and call for EMS using the built in LSPDFR backup menu, what happens? Is it possible you've modified files in your gta v/lspdfr/data folder, causing there to be no valid ambulance entry?
 
 
 
Can you please confirm that this was with no other mods running and no LML packages installed? Is your GTA installation on a physical drive inside your computer (not running off a USB drive)?
