I went to review a file and it keeps saying that I need to download it first before reviewing, except I've already downloaded it. What's up with that?

Does it happen on all files, or just the one?  If it's just the one, please link it.

It's happening sporadically. It'll occur on a few files, then on none, then on all of them, etc.

The only time I've had this happen to me is if I clicked download, there's a captcha, and I go back to the file page instead of filling it out.  Cyan might have more information on this.
