I'm not sure what this means, however, the issue might have something to do with your callouts.  Can you please list the callout packs you have?

If you look at the download page for SWAT callouts, he said the plugin is very unstable.  I'd start by temporarily removing this plugin and see if it works.

I'm not positive on what that means, but where is your GTA V folder located?  C drive, or an external drive?
