The latest updated version of RAGE Plugin Hook, required to play LSPDFR, can be found inside the LSPDFR download. It is not currently available on the RAGE Plugin Hook website.

When I install a car mod I install just like it says in the read me but when i get in game and try to spawn it it shows up as the original GTA vehicle. Help

Put the modded car in the very latest patchday that has a vehicles.rpf.  At some point, an update made it to where using different patchdays doesn't work anymore.

Yes.  You'll have to copy each dlc pack to it.

Double click on the .yft of your modded car.  It will open a viewer so you can see the car.  Is this your modded vehicle? or is it the vanilla GTA vehicle?
