Well you have a few options.

 

The most easiest, but requiring the most space, is to copy your GTAIV installation to an external media such as a USB Stick. Please note that this USB stick will require around 16-20 GB.

 

If you want to make backups of the most important files, like default vehicles, textures, etc, then you should either
Export each vehicle or texture to a reliable external media or another place within your computer or make backups of the .img files.

The most important .img files are your vehicles and ped_ ones. 

To do so, go to

PC -> Models -> CDI Images

backup those .img files.

That way if you ever have an accident and you need to replace your car files, you should do that.

 

Another good tip is to put in some working uniforms and cars that you like, then make a backup of that. So if something fails, you can revert back to what you previously had. 

To save space, put them all in RAR/Zip files. Extract them when you need to at a later date.
