Program/tool for showing and launching different GTA4 directories?

I don't know if the title says what I mean, so...

I'm looking for a program that shows GTA4 directories and lets you launch any one you want. I don't want to have a lot of shortcuts on my desktop for about 15 folders different mods installed (yep..) so I thought such a tool could be useful.

 

Does anyone know something similar or is someone able to make this?

If you have Steam, you can add "Non-Steam Games" which are basically any programs you have on your PC that can have Steam set a shortcut to. I have multiple GTA IV games on my Steam directory and it allows me to play them with the click of a button.
