Sirens are Wail, Yelp and pier (piercer) or Hi-Lo (Two-tones - NEEENAAAA), that's the different tones that are used by most emergency services.

Since GTA IV isn't focused on emergency services from the developers, it works like that, you can in theory mess around with it as much as possible using IVAUD.

The reason for it being like that is, well it needed a limit, you are not meant to drive about in police cars all day, Rockstar did not intend it to be like that, in theory someone could script it so you could use more than just three tones and a air horn, but having three tones is adequate.
