What's the difference between doing F12 for backup or ALT+B? It seems neither respond too well, but sometimes I'll get some good backup.

And whenever I call noose they never seem to show up or they take forever.

F12 is backup when you are in a pursuit. F12 backup will not work unless you are in a pursuit. ALT + B, on the other hand, is for backup when you are not in a pursuit, e.g. when you have responded to a call and need some extra help.

As for the NOOSE thing, the same thing happens to me sometimes. Not too sure why but it doesn't bother me because I just call for more NOOSE units.

ALT+B uses GTA IV's "Call Backup" from the original police computer (spawns a car which responds to your current location), F12 spawns a car which will join your pursuit, not only drive to a location.
