I am currently running two monitors, when I keep the second one plugged in, and my game on full screen, I can open my computer in my car, and the on-screen objects(the login area) will not be centered so I have to guess where the buttons would be. Same with policehelper, it goes to (right of the middle, left of the normal position) and I have to guess where the buttons would be. Now if I un-plug my second monitor, this isn't an issue.

Set your GTA IV aspect resolution to 16:9 rather than automatic. (assuming you have a 16:9 monitor)

 

That will stop GTA IV from checking the aspect ration and changing it, and hopefully will fix your issue. GTA IV was never built to work on 2 monitors. I have issues with it too :)
