It would help immensly if you could answer these questions. Legal copy of the game? Running the game as administrator? Latest patch? What other mods are you using? What operating system are you using (Windows XP is NOT supported)? Make sure that LCPDFR 0.95 is removed (http://www.lcpdfr.com/page/tutorials/index.action/_/script-data-file-modification-tutorials/how-to-uninstall-lcpdfr-r19).

Running the game as administrator: Go to the GTA IV folder, right click on GTAIV.exe, Properties, Compatibility and select 'Run this program as administrator', do the same exact thing to the LaunchGTAIV.exe.

 

Are u using modded/ custom PEDS and/ or components? I had a similar issue that was corrected by using the default PEDS. This is a known issue with the developers. Usually occurs when you enter the PD station to go on duty. It was thought that you could avoid the issue by not going in the PD station and instead using the "forceduty" command in the console. However, the issue would still arise within 5 minutes of gameplay...

The error logs for my support request seem similar to yours:
