It's a Federal Q siren mixed with the wail of another Federal Signal siren (I'm pretty sure it's a FedSig wail, it sounds like one), I don't know the models of either siren though.

No problem, there are a few lurking around on YouTube and such. If you're looking for an accurate one for GTA IV though you're gonna have a hard time, since GTA IV restricts the length of the tones to 6 seconds or less. So far nobody has been able to make a decent sounding one within that time frame.
