Hi I am having the same problems as well.

I had restart my computer and holded down the LShift key until the RageHook window comes up. The I went over to the Plugins Tab click on it, it show all my plugins were all checked off to include the EUPMenuu as well.  I had resave it again. When Ragehook rebooted itself it show that EUPMenu was loading up as well. There no error after it has been loaed at that time and while in game as well. 
But when I had press F4 and brought its window, right below I was going to load the EUPMenu one more time to make sure its loaded but it wasn't there to chose it. It had this one to choose; EUPMenuReloadAllConfigs.
When I press F11 for the EUPMenu it brings up a different window. All my Firing walls are disable and my windows defender as well. 

I need some HELP cause I am not sure of what I am doing wrong!  I have followed Criminal Justices in How to do a complete install of the LSPDFR and Plugins to the letter.
I have enclosed some screenshots, and my GTAV Log as well.

You dont need to load EUP a second time, in your log its automatically loaded.

What menu is opening when you press F11? Post a screenshot, for example. My guess is another plugin has the same key for the menu. This can be changed in the .ini settings file.
