File update pending approval bug?

Hi @Cyan,

 

I updated this file earlier today -

 

But it seems like the file bugged?  It never said anything about the fact that I updated it.  When I click file actions, I see this:

Is it actually pending mod approval and there's no orange notification message, or what happened?
