    I am currently compiling a form for an arrest report for the LSPDFR. And I could use help with two things. 1. Think of something to fill in the remaining gray field. 2. Since English is not my native language, I most likely have errors here that need to be corrected. This is especially true for the meaning of words. After I correct the errors and conduct a little testing, I will post the source available for editing. And thanks in advance. (And I apologize if I wrote in the wrong forum thread and for possible errors)
