    Found the issue:

    3 add-on modifications from ThinLineSanctuary (Specifically Axon Body 4, Guardian Load Bearing Vest, and add-on belts caused this issue. Removing them fixed it immediately. Will reach out to TLS himself.
  2. Nocturne's post in Help with rph crash report was marked as the answer   
    The plugin: LSPD First Response should not be loaded more than once. Open the startup.rphs file in your GTAV folder and remove its content. Save and close the file and test again.
