Solutions

  1. Yazzar's post in LSPDFR Crashing on Callout - Slimdx. Crash to Desktop - again! was marked as the answer   
    Turns out it was an issue with my CORSAIR Gaming Headset - when I switched to speakers everything fixed itself.
     
    I then had to go on the Corsair GUI and switch audio from Stereo to 7.1 Surround Sound and everything works fine now.
