 Delete the GTA IV folder, then clear the AppData => Local and Roaming folders belonging to GTA IV and then clear the HKEY_LOCAL_MACHINE => SOFTWARE registry keys belonging to GTA IV and you're done. It's a weird issue with Windows XP.

Press WindowsButton + R,  type 

%appdata%

Go back one step, look for folders named Local and Roaming. Inside those, look for folders belonging (read=named) to R* or GTA IV. 

 

For the registry, again press WindowsButton + R,  but this time type 

regedit

Look for HKEY_LOCAL_MACHINE. In that entry, there should be a folder called SOFTWARE . Find the R* and GTA IV related stuff and delete them.

 

Needless to say: MAKE BACKUPS BEFORE DOING ANYTHING.
