The latest updated version of RAGE Plugin Hook, required to play LSPDFR, can be found inside the LSPDFR download. It is not currently available on the RAGE Plugin Hook website.

Do the following, reasons will be listed:
Remove Callout Manager, it is outdated and replaced by vanilla features
Remove LP Callouts, it is outdated
Update Super callouts, you are on 2.6.0 and the latest is 2.7.1

Try without Safer Chases, it is outdated and can cause instability

In Rich's police enhancements set EnablePriorityRadioTraffic to false, it requires Police Smart Radio to work. This is what your log says is causing the crash, the others are just for stability.
