I have a tip that might help... Set your Default GTAIV siren key to your middle mouse button, so when driving, you can play with your sirens without taking your hand off the mouse...

 

This was a great idea. I set the default G to my middle mouse button and I can toggle through the variety of sirens. Thanks.

    open up the game to play.... before starting, go to options/controls.... look around in there. Look for the Horn Button

    Of course..duh... LOL
