Solutions

  1. trocapalot's post in LSPDFR Crash when accepting a callout due to SlimDX was marked as the answer   
    This seems to only be happening when receiving any callouts from Kuchera Callouts which is odd as it was working completely fine earlier.
     
    I fixed it by replacing the scanner audio files, for whatever reason the old ones we're causing crashes
