You need to give it a second to show up.  The more chest compressions you give, the longer it'll take for the bar to come up.  You need to do two or three compressions, stop, and wait for the bar, then continue.

I have DirectX11 and Forced Windowed settings on but that shouldn't be an issue right?

It shouldn't make a difference, but try without windowed mode.  If still nothing, try asking the mod author.
