Crashing when shot and killed

Every time my officer is killed by a gunshot LSPDFR crashes.  GTA V then crashes when trying to spawn the office again at the hospital.

If I'm killed through other means I respawn fine, however every time I'm shot it's game over, meaning I have to reload GTA V.

I don't think it's a mod related issue as it happened prior to any mod's being installed and still happened when I removed them.

Any ideas on how to fix?

My system is;

O/S: Windows 10 Pro

CPU: i7 6700k - 4.4 GHz OC (it was 4GHz standard until today - The issue happened before this)

GPU: AMD R9 380x

MB: Gigabyte Z170 Gaming K3

2x Samsung SSD 250GB Raid 0 & 1x Seagate HDD 2TB

Memory: DDR4 16GB

The game is installed on my SSD opposed to my HDD as I didn't want the disk spinning up all the time I'm playing.  Other games / apps are installed to the HDD.
