Does anyone know what kind of light bar this is and/or have an unlocked model?

Looks kinda like a Whelen Liberty with colored glass. Although, I am probably wrong (sorry, I'm tired).

Oh wow. Thanks. I already had this light bar. I am making it with the colored glass as requested by my friend who really wants the light bar in the picture.

I realized after that the back is probably yellow lights which will be incorporated. I'm not sure why the front glass is white. I looked at yours and a real picture, which they look the same.

Yup as previously stated it is a Whelen Liberty with colored glass on the left and right LEDs. Here is a picture that shows the front of Lousiville's car.
