The latest updated version of RAGE Plugin Hook, required to play LSPDFR, can be found inside the LSPDFR download. It is not currently available on the RAGE Plugin Hook website.

Launch GTAV without LSPDFR loaded and rename your mods folder to _mods if you have one. If GTA loads fine without LSPDFR, reinstall LSPDFR using the manual installation and/or undo any recent changes you have done to your mods folder, or entirely delete and redo your mods folder. 

 

If this does not work, it indicates a faulty disk drive / GTAV install. You should follow this Microsoft guide to troubleshoot disk failures.
