Hopefully someone can help, because my frustration level is HIGH.  So I've installed GTA V (fresh install from steam today), and have installed LSPDFR and the Rage Hook.  The game initiates fine on it's own, and I can then initiate the Rage Hook with the game already running and it loads in fine.  I then try to load in the LSPDFR plugin, and it looks like it's successful - it changes the character - but then immediately crashes.  I've had it working twice, both times after a reboot.  I've fiddled and fiddled, even clean installed both LSPDFR and Rage Hook a couple of times.  Please help.

Do you have already try reinstall the LSPDFR?

Yes, multiple times.  My drivers are up to date as well.

If it helps, I've since discovered it'll run after a fresh reboot.  But if the game closes out for any reason, it will not run when you reload the game.
